Your First Visit

In order to ensure your first visit is as short and smooth as possible, please bring the following with you to your appointment at TMJ & Sleep Apnea Help Center in Glendale:

  • Your completed patient information form if mailed to you unless completed online.
  • Your insurance card and photo identification.
  • Any X-Rays, scans or radiographs you have had made by other doctors.

We ask for your cooperation in making and keeping all appointments. Our office policy requires a two-business day notice to change appointments to avoid a $75 broken appointment fee.
